I am participating voluntarily and at my own risk.*

I understand that I am responsible for monitoring my own physical and mental condition during participation.*

If I experience pain or discomfort during the session, I will immediately inform my therapist. I will not hold my therapist responsible for any pain, discomfort or distress I experience before, during or after the session.*

I acknowledge that I should consult an appropriate healthcare professional if I have concerns regarding participation with any medical condition, injury or limitation.*

All payments are due in full in advance of the first session, or upon arrival at first session. *

All session fees (casual, plans, packages and memberships) are non-refundable.*

Clients may reschedule sessions online up to 12 hrs prior to session start time, or cancel/reschedule with minimum 4 hours notice, by directly contacting Dr Steve.*

Sessions may be relocated or rescheduled due to, severe weather or unsafe conditions, including:

·        heavy rain

·        thunderstorms

·        strong wind

·        extreme heat

·        severe weather warnings

or, illness or other unavoidable absence of instructor.*
